This page can be used by individual members of the public to self-refer, or by statutory and professional agents making a referral on someone’s behalf.
Art Medicine brings together creative healing, holistic wellbeing, reflective practice and supportive pathways rooted in dignity, culture, care and transformation.
Please use this form to tell us who the referral is for, what kind of support is being sought, and how we can make safe and appropriate contact.
If you are referring somebody else, please ensure you have their knowledge and consent wherever it is safe and possible to do so.
This form is not for urgent or emergency support.
If someone is in immediate danger, at risk of harm, or needs urgent mental or physical health support, please contact emergency services, a GP, crisis team, social care duty team or other urgent safeguarding route as appropriate.
