Make a referral
This page can be used by individual members of the public to self-refer, or by statutory and professional agents making a referral on someone’s behalf.
Art Medicine brings together creative healing, holistic wellbeing, reflective practice and supportive pathways rooted in dignity, culture, care and transformation.
Please use this form to tell us who the referral is for, what kind of support is being sought, and how we can make safe and appropriate contact.
If you are referring somebody else, please ensure you have their knowledge and consent wherever it is safe and possible to do so.
This form is not for urgent or emergency support.
If someone is in immediate danger, at risk of harm, or needs urgent mental or physical health support, please contact emergency services, a GP, crisis team, social care duty team or other urgent safeguarding route as appropriate.
Send referral details
Use the form below to make a self-referral, refer someone with their knowledge where appropriate, or begin a conversation about support, suitability and next steps.
Please share as much relevant detail as you can. Clear information helps us respond more thoughtfully and safely.
Who this is for
Individuals, family members, carers, advocates, statutory partners, educators, support workers and other professionals seeking a thoughtful referral route into Art Medicine.
The type of support being sought, relevant background, any access needs, who is making the referral, and the safest way to make contact.
Submitting this form does not replace urgent safeguarding, crisis or medical support. Immediate risk should always be directed through an urgent or emergency route.
Healing can begin with the right invitation, offered with care.
A gentle and considered route
Referrals are approached with attention to wellbeing, dignity and context. The aim is not simply to process information, but to understand what kind of support may be most appropriate and how contact can be made in a safe and respectful way.
After the form is sent
Once the referral is received, the information will be reviewed and a response will be made as soon as possible. In some cases, further detail may be needed before the most suitable next step can be offered.
Care, creativity and contact
This referral page is here to make first contact feel clearer, calmer and more held. Whether the referral is for you or for somebody else, it offers a respectful starting point for conversation, support and possibility.
